01Who it's for
Open to most Ontario residents with sports or injury concerns.
- Have a sports or activity-related injury (>1 week)
- Are returning to sport after an injury
- Have a concussion in the last 12 months
- Are training for an event (Hyrox, marathon, etc.)
- Want structured performance medicine support
- Are an Ontario resident with a valid health card
02How the program works
Initial Intake & Goal Setting
Describe your injury or performance goal. For injuries: when did it start, how does it feel, what makes it worse? For performance: what event are you training for?
Imaging or Lab Work if Needed
X-rays, MRI, or labs (if relevant — e.g. iron studies for endurance fatigue) ordered as appropriate.
Specialist Consultation
A virtual visit to assess history, demonstrate movement testing remotely (often surprisingly informative), and design your plan.
Plan Delivery
A written plan: phase-by-phase progression, rehab exercises, return-to-sport milestones, prevention strategies. Coordinated with physiotherapy if needed.
Follow-Up & Adjustment
Regular check-ins to assess progress, adjust loading, and progress through phases. Same physician each visit.

04Coverage & what's billed
What's covered: physician consultations, requisitioned lab work, prescriptions written by your DOS physician, and follow-up visits during the program.
What may be patient-pay: certain medications (depending on private insurance), devices, supplements, and any out-of-program services you elect. Your DOS physician will discuss any patient-pay item with you before prescribing — nothing is added to your file without consent.
05Your pharmacy
Your preferred Rexall pharmacy can assist with prescription fulfillment, medication availability, general pharmacy-related questions and product inventory inquiries. Clinical assessments, diagnoses, prescribing decisions and treatment plan modifications remain the responsibility of your physician.

Open to most Ontario residents with sports or injury concerns.
This program may not be right for you if you:
- Have an acute traumatic injury requiring ER care
- Have suspected fracture needing immediate imaging
- Need same-day in-person musculoskeletal exam
